{"id":7488,"date":"2021-03-03T14:20:34","date_gmt":"2021-03-03T13:20:34","guid":{"rendered":"https:\/\/science-nutrition.com\/?p=7488"},"modified":"2021-03-09T10:45:43","modified_gmt":"2021-03-09T09:45:43","slug":"point-sur-la-vitamine-d-et-la-covid-19-en-europe","status":"publish","type":"post","link":"https:\/\/science-nutrition.com\/en\/2021\/03\/03\/point-sur-la-vitamine-d-et-la-covid-19-en-europe\/","title":{"rendered":"Focus on &#8230; vitamin D and COVID 19 in Europe"},"content":{"rendered":"<div class=\"l-subsection\"><div class=\"l-subsection-h\"><div class=\"l-subsection-hh g-html i-cf\">\n<div class=\"subtitle\"><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b ChMk0b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Ensuring satisfactory vitamin D status in the general population is particularly important in the context of Covid-19, and numerous clinical studies on the impact of vitamin D on COVID-19 infection have been initiated across the world.<\/span> <span class=\"JLqJ4b ChMk0b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"1\">To date, only 14 clinical studies on the subject have been completed (including 2 in Europe: 1 in Spain and 1 in France, <a href=\"https:\/\/clinicaltrials.gov\/ct2\/results?term=vitamin+D&amp;cond=Covid19&amp;Search=Apply&amp;recrs=h&amp;recrs=e&amp;age_v=&amp;gndr=&amp;type=&amp;rslt=\" target=\"_blank\" rel=\"noopener noreferrer\">clinicaltrial<\/a>).<\/span><\/span><\/div>\n<ul>\n<li>\n<div class=\"J0lOec\"><strong><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b ChMk0b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Vitamin D supplementation contributes to the proper functioning of the immune system and helps prevent infection with COVID 19:<\/span><\/span><\/strong><\/div>\n<\/li>\n<\/ul>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">The first reports show that the <strong>circulating concentration of 25 (OH) D is lower in infected adults than in others<\/strong>. In an ecological study, inverse correlations were found in 46 countries between vitamin D deficiency in the general population and the incidence of Covid-19. Specifically, the existence of vitamin D insufficiency appears to precede the incidental occurrence of Covid-19, and not the other way around. <strong>Despite the lack of solid intervention data at this time<\/strong>, preliminary results from the <a href=\"https:\/\/oslo-universitetssykehus.no\/om-oss\/nyheter\/kan-tran-forebygge-korona\" target=\"_blank\" rel=\"noopener noreferrer\">Koronastudien.no study<\/a>, showing in Norway that regular consumers of cod liver oil are less likely to be infected with SARS-CoV -2, suggest that vitamin D supplementation could help prevent Covid-19. <\/span><\/span><\/p>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\"><strong>The few published intervention studies appear to support a beneficial effect of vitamin D supplementation<\/strong> in reducing the severity of symptoms in adults with Covid-19, but the number of studies remains too small.<\/span><\/span><\/p>\n<ul>\n<li>\n<div class=\"J0lOec\"><strong><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b ChMk0b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Vitamin D does not help cure patients with severe COVID 19:<\/span><\/span><\/strong><\/div>\n<\/li>\n<\/ul>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">A r<a href=\"https:\/\/nutrition.bmj.com\/content\/early\/2021\/01\/07\/bmjnph-2020-000151\" target=\"_blank\" rel=\"noopener noreferrer\">ecent Mendelian observational study<\/a> suggests that vitamin D does not prevent severe forms of Covid-19. An article published in the <a href=\"http:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2775003\" target=\"_blank\" rel=\"noopener noreferrer\">Journal of American Medicine Association<\/a> also criticizes the craze around vitamin D. <a href=\"https:\/\/www.who.int\/elena\/titles\/commentary\/vitamind_pneumonia_children\/fr\/\" target=\"_blank\" rel=\"noopener noreferrer\">A 2017 review<\/a> carried out by the World Health Organization (WHO) on vitamin D and respiratory infections was a reminder of how much the level of evidence was weak and studies heterogeneous, making conclusions difficult. A recent article published in <a href=\"http:\/\/academic.oup.com\/nutritionreviews\/article\/79\/2\/200\/5873314\" target=\"_blank\" rel=\"noopener noreferrer\">Nutrition Reviews<\/a> evokes a plausible utility in people who are really deficient (less than 10 nanograms per milliliters) and those at high risk of going through the intensive care unit if they contract Covid-19 (which generally represent the same sub- groups). Taken together, these data constitute an unconvincing body of evidence to support claims to supplement the general population with prevention purposes. <\/span><\/span><\/p>\n<p><strong><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">It seems that the recommendation to ensure, for the purpose of primary prevention of COVID-19, that the blood values \u200b\u200bof 25 (OH) D are aligned with those proposed by the Haute Autorit\u00e9 de Sant\u00e9 &#8211; 20 ng \/ mL for the general population &#8211; or the Group for Research and Information on Osteoporosis (GRIO) -30 ng \/ mL for the elderly- may be justified in view of the meta-analyzes relating to the prevention of acute respiratory infections (in the absence of the specific context of COVID-19). On condition, as these meta-analyzes indicate, to favor, in the event of a proven deficit (less than 20 ng \/ mL), daily or, possibly, weekly supplementation (<a href=\"https:\/\/www.vidal.fr\/actualites\/26529-vitamine-d-et-covid-19-la-supplementation-presente-t-elle-un-interet.html\" target=\"_blank\" rel=\"noopener noreferrer\">Vidal.fr<\/a>).<\/span><\/span> <\/strong><\/p>\n<ul>\n<li><strong><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b ChMk0b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Opinion of the European countries:<\/span><\/span><\/strong><\/li>\n<\/ul>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Our British neighbors have made the decision to use vitamin D. Indeed, the <strong>governments of the <a href=\"https:\/\/www.gov.uk\/government\/publications\/vitamin-d-for-vulnerable-groups\/vitamin-d-and-clinically-extremely-vulnerable-cev-guidance\" target=\"_blank\" rel=\"noopener noreferrer\">United Kingdom<\/a> (<a href=\"https:\/\/www.euronews.com\/2020\/11\/28\/coronavirus-2-5-million-of-most-vulnerable-people-in-england-to-be-sent-vitamin-d-pills\" target=\"_blank\" rel=\"noopener noreferrer\">England<\/a>, <a href=\"https:\/\/www.gov.scot\/publications\/vitamin-d-advice-for-all-age-groups\/\" target=\"_blank\" rel=\"noopener noreferrer\">Scotland<\/a> and <a href=\"https:\/\/www.irishtimes.com\/news\/health\/minister-urged-to-recommend-vitamin-d-supplements-in-covid-19-strategy-1.4485237\" target=\"_blank\" rel=\"noopener noreferrer\">Ireland<\/a>) have for their part planned to provide vitamin D as a preventive measure to several million fragile people, to elderly hospitalized people<\/strong>, based in particular on the good results observed in <a href=\"https:\/\/www.euroweeklynews.com\/2020\/11\/15\/spains-andalucia-to-provide-vitamin-d-in-residences-after-studies-show-it-helps-combat-the-coronavirus\/\" target=\"_blank\" rel=\"noopener noreferrer\">Andalusia<\/a>. <\/span><\/span><\/p>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">The Irish strongly encourage the recommendation of vitamin D supplementation to prevent infection, due to their inherent deficiency in the country&#8217;s low sunshine. In their <a href=\"http:\/\/www.imj.ie\/wp-content\/uploads\/2020\/04\/Optimisation-of-Vitamin-D-Status-for-Enhanced-Immuno-protection-Against-Covid-19.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">report<\/a>, The Irish Government recommends that all elderly people, hospital patients, nursing home residents and other vulnerable groups urgently receive a supplement of 20 to 50 \u03bcg \/ d of vitamin D to improve their resistance to Covid-19, and that this advice be quickly extended to the general adult population. <\/span><\/span><\/p>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\"><strong>Belgium<\/strong> concludes in its <a href=\"https:\/\/www.health.belgium.be\/sites\/default\/files\/uploads\/fields\/fpshealth_theme_file\/210119_css-9620_vitd_zn_covid_vweb.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">report<\/a>: &#8220;neither zinc nor vitamin D are the panacea in the treatment of Covid-19. The use of very high amounts of vitamin D for therapeutic purposes in patients with Covid-19 and to prevent very severe forms of the disease, cannot currently be recommended. &#8220;<strong><a href=\"https:\/\/www.iss.it\/documents\/20126\/0\/Rapporto+ISS+COVID-19+51_2020.pdf\/01dcd756-8cce-0b08-6573-be67872cded1?t=1592838400233\" target=\"_blank\" rel=\"noopener noreferrer\"> Italy<\/a>, <a href=\"https:\/\/www.mscbs.gob.es\/profesionales\/saludPublica\/ccayes\/alertasActual\/nCov\/documentos.htm\" target=\"_blank\" rel=\"noopener noreferrer\">Spain<\/a> and <a href=\"https:\/\/www.dge.de\/uploads\/media\/DGE_Fachinfo_VitaminD_COVID-19_Stand_Januar_2021.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Germany<\/a><\/strong> have looked into and interested in the subject, however <strong>they consider the current evidence insufficient to make a recommendation<\/strong> for the general population and fear possible risks associated with potential vitamin D overdoses. <\/span><\/span><\/p>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">In <strong>France<\/strong>, the Academy of Medicine also recommends vitamin D supplementation (<a href=\"https:\/\/www.academie-medecine.fr\/communique-de-lacademie-nationale-de-medecine-vitamine-d-et-covid-19\/\" target=\"_blank\" rel=\"noopener noreferrer\">Press release from the Academy of Medicine, May 22, 2020<\/a>).<\/span><\/span><\/p>\n<ul>\n<li><strong><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b ChMk0b C1N51c\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Need for a consensus on the reference values for Vitamin D:<\/span><\/span><\/strong><\/li>\n<\/ul>\n<div><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">Disagreements remain on the <strong>interpretation of blood levels of 25 (OH) D<\/strong> (25-hydroxyvitamin D, the usually dosed form). Without entering into the scientific debate on this subject, we will simply recall the values \u200b\u200bthat are commonly used in practice in France: <\/span><\/span><\/div>\n<ul>\n<li><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\"><strong>in the general population<\/strong>, a blood concentration of 25 (OH) D greater than <strong>20 ng \/ mL<\/strong> (50 nmol \/ L) is considered &#8220;sufficient&#8221;, <\/span><\/span><\/li>\n<li><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">in<strong> the population at risk<\/strong> of age-related osteoporosis, a disease or a chronic treatment, this reference blood level should, according to the recommendations of the GRIO , be greater than <strong>30 ng \/ mL<\/strong> (75 nmol \/ L). <\/span><\/span><\/li>\n<\/ul>\n<p><span class=\"VIiyi\" lang=\"en\"><span class=\"JLqJ4b\" data-language-for-alternatives=\"en\" data-language-to-translate-into=\"fr\" data-phrase-index=\"0\">In terms of toxicity, it is estimated that the risk of adverse effects (hypercalcemia, kidney stones, for example) appears when blood levels persistently exceed 50-60 ng \/ mL According to the National Agency for Food, Environmental and Occupational Health Safety (ANSES), the daily intake of vitamin D should be 15 micrograms (600 IU) per day for an adult .
